
There are several pathways for people to be eligible and access SPDL services.

HOW TO USE OUR SERVICES

You are 65 years of age or over.
You must be registered with My Aged Care and be on a Commonwealth Home Support Package (CHSP) or Home Care Package (HCP).
Get a referral to SPDL via My Aged Care (1800 200 422).

You must be under 65 years of age.
You can self-refer to us or via Care Provider/The City of Port Phillip.

You must be under 65 years of age.
Get referred through yourself (if you manage your NDIS package) or your NDIS Provider.

HOW SPDL CAN HELP YOU

DayLinks provides door-to-door community transport service within City of Port Phillip and City of Melbourne.
DayLinks service drives clients to GP visit, hospital appointments and essential errands — trips that other services like taxis or ride-share companies may not accommodate due to the short distances many of the trips our clients need.

This program links volunteers who enjoy gardening with older people who need support to maintain their garden.
Please note:
The service does not provide lawn mowing, rubbish removal or tree pruning. -
The PetMates program assists older residents with the care of their animals at home with grooming, dog walking and/or visits to the vet.

This program links a suitable volunteer to support older residents who need encouragement and company to go for a walk to maintain their physical fitness.
